The following table lists general troubleshooting tips.
If you did this... |
Here’s what you can do... |
---|---|
DRG 999 - Ungroupable in ChartWise |
Here are the most common reasons for DRG 999 and how to resolve them:
|
I sent the query to the wrong physician. |
Reassign the query - See Change the Query Recipient and/or the Query Reason. |
I responded to the wrong query. |
Delete the query - See Delete a Query and then initiate a new query. |
I responded to the correct query but for the wrong physician. |
Delete the query and initiate another query, then respond to the query on behalf of the correct physician. See Queries. |
I can’t find the code I’m looking for. |
Ensure that the appropriate option (Diagnoses or Procedures) has been selected in the Search box drop-down list. See Add a Diagnosis or Procedure.
Check the admission and discharge dates to be sure that they aren’t so old as to be using an older codeset version. TruCode presents codes for the two most recent codeset versions. |
I find the code but I can’t add it to the admission. |
Ensure that it is a valid code and a plus sign appears in front of the code when you hover your cursor over the number. Some codes require further specificity. See Add a Diagnosis or Procedure. |
I changed the payer and ICD-9 codes were deleted. |
This indicates that the new payer uses a different codeset version than the old payer, so those codes are no longer valid. You will have to re-enter the codes in the correct codeset version. |
I changed some demographic information and some of my ICD-9 codes were deleted. |
This indicates that the new demographic information makes certain ICD codes not applicable to this patient and were removed. |
I am working in Dual Group MS DRG and I am not seeing anything in the SOI or ROM fields. |
Click the Dual Group button. They will be populated without clicking the button, but it may take the system up to 24 hours to do so. |
I have an unreconciled code (orange colored) in the Billed state, how do I get it to reconcile? |
There may be a delay in reconciling an orange code bubble, but the system will reconcile that code if it has been verified in a previous state of the documentation. |
Some of my code bubbles are gray colored. What can I do to change their color? |
Gray colored code bubbles mean that the admission has been imported from a previous version of the software. If you click the Push Baseline to Working button, you should see the code bubbles’ correct colors. |
I worked on a chart but it doesn’t show up in the Recent Activity feed on my Home page. |
Whenever you work on a chart, you must always click the Review icon and make a selection for your review to be recognized for reporting purposes and to be displayed in the Recent Activity feed. |
I worked on a chart 2 months ago...how can I find it again? |
Use the Account Number search box in the main banner of the application. |
I got an error message that my search returned more than 100 entries and I need to refine my search. What do I do? |
Use the filters at the top of the page to narrow your search down to exactly the admissions you are looking for. |
I’m trying to enter one of the following: a procedure for a specific date, a discharge date, a Next Review date/time, an effective date for labs, medications or findings, a query, a query response and it’s not letting me. What do I do? |
First you should check that the date you are entering for any of these events is not before the admission date, or in the case of a query response that the date is not before the query creation date. You also can’t select a Next Review date that is before the current date. |
When I was adding a diagnosis I get the following warning message: “Unable to add duplicate code (x)” where x is the code in question. |
Only a single click is needed when adding diagnoses. If you double-click, you will get this warning. |
