The Query Details box to the right of the query form shows you the following information about the selected query:
Date and time the query was created.
Who created the query.
Date and time the query was published (in other words, the date and time that the Send button was clicked).
The query recipient (if more than one recipient has been designated, this field will display as "Providers").
The query acknowledger (once the query has been resolved).
Whether or not the query has been scanned/uploaded into the EMR.
Whether or not a response has been noted in another location such as the Progress Notes, Discharge Summary, and so on. This indicator is for the CDS to mark when they have verified the physician has documented the answer to the query in the medical record in addition to answering the query. It is intended to be used as a quality check for those facilities that require the physician to carry the diagnosis forward in the medical record.
Whether or not the query has been sent electronically.
Whether or not the query recipient has been provided with education related to the query topic or contents.
The reason for the query.
The query impact.
The query type (Written or Verbal).
Diagnoses & procedures queried from.
Diagnoses & procedures generated by the query (including the +Code button to add a generated code to a resolved query).
To change the recipient of the query, click the button displaying the current Query Recipient’s name and the following fields display:
Make changes to the recipient(s), as needed, and then click the Reassign button. If the query had already been sent to the original recipient(s), clicking the Reassign button will automatically send the query to the new recipient(s). If the query is still in a "Draft" status (in other words, has not yet been sent to the original recipients(s)) then no notification of reassignment will be sent. Once the query is ready, click the Send button and it will be sent to the new assigned recipient(s).
From the Query Details box, you can change the scan/upload status of the query by selecting or deselecting the Scanned into EMR checkbox, as well as change whether or not the response has been noted by the physician in some other location by selecting/deselecting the Response Noted checkbox.
You cannot change the Sent Electronically checkbox as it is set by the system when a query has been “Published” (in other words, the Send button has been clicked).
Select the Education Provided checkbox if you needed to assist the Provider with answering this query.
As an option, your site can choose to have the Education Provided checkbox appear as a dropdown box containing a list of education reasons, as follows:

Other
This information is then recorded for reporting purposes.
You can change the reason for querying and the query impact from the Query Details box as well. Simply deselect and select the appropriate checkboxes.
To change the query type, click on the appropriate toggle button: either
Written or Verbal, as follows:
When you create a query from the Queries tab, you may want to associate that query with a code you have added on the Codes tab. That way, when the query is answered, it will change the associated code from Speculative to Confirmed and, therefore, billable.
To associate a query to a code, click the Select Code link in the Query Details box. The Possible Codes box displays, as follows:
The Possible Codes box shows you all of the Diagnosis
and Procedure codes for this admission. Select the appropriate code to
associate with this query by clicking on the icon.
You can delete the association by clicking on the
icon to the right of the code in the Queried From box.
If a query was resolved without a particular code associated to it,
you can associate that code to the closed query by clicking the
button in the Generated Codes box of the Query Details section. ChartWise
2.0 will open the TruCode Encoder within the Queries tab to allow you
to search for the diagnosis or procedure code that should result from
this closed query. Once you have located the appropriate code or multiple
codes and added them, click the Close Encoder button to return to the
Queries tab. A PDF of the query displays on the tab, and the Generated
Codes box will now show the diagnosis or procedure code(s) that you associated
to that query.
